Using Insurance for visits and labs

Lab tests and imaging studies ordered by an M.D. are also usually approved and covered by PPO insurance and Medicare.  Labs by non-M.D.s are not usually covered, which might require you to pay more out of pocket.   Reimbursement is based on the terms and conditions of your plan for lab work.  You might have a deductible plus a co-pay.   Please keep in mind that specialized functional medicine lab tests will not be covered by insurance.
